From the main campus website, use the Quick Links section in the top right hand corner.
From Quick Links go to the E-Forms option.
On the E-Forms page you will log in like you would a campus PC.
(ex. John Doe with an ID # of 1234567 and birthday of March 18th, 2002 would use jd1234567 as the username, and Jd031802 for the password.)
Click on the COVID 19 link to start your form
On the first page of the form, you will input the type of vaccine, and the date of the final dose as well as if you are a student or an employee.
(If you are both, choose employee)
On the next page you will submit an image of your Immunization records be it a screenshot, scanned image, or photo.
Once you have the image there, you will click the Attach button, and it will submit your form.
You will be taken to a summary page, and you should get an email sent to your campus email address for confirmation as well.
